4. Cancellation Policy

We request a minimum of 24 hours notice for cancellations or rescheduling. Appointments cancelled with less than 24 hours notice may be subject to a late cancellation fee of up to $75. No-shows without prior notice may be charged the full consultation fee.

We understand that emergencies happen. Please call us as early as possible at (555) 123-4567 so we can accommodate other patients on the waiting list.

5. Payment Terms

Payment is due at the time of service unless a prior arrangement has been made in writing. We accept major credit cards, debit cards, and cash. We work with most major dental insurance providers. Patients are responsible for any balances not covered by insurance.

Treatment plan estimates are provided in good faith but are subject to change based on clinical findings during treatment. Any changes will be communicated to you before proceeding.
